Not every foot or ankle problem shows clearly on an X-ray. While X-rays are excellent for evaluating bones, fractures, heel spurs, arthritis, and structural deformities, many painful conditions involve soft tissues such as tendons, ligaments, nerves, fascia, muscles, cysts, and inflamed joint structures.

Diagnostic Ultrasound gives podiatrists a real-time view of soft tissue movement and injury, helping identify the cause of pain more precisely. Ultrasound uses sound waves to create images of structures inside the body and can help guide diagnosis and treatment for many conditions.

At Comprehensive Podiatry Associates, P.C. Foot Specialists, patients in East Setauket, Selden, Stony Brook, Port Jefferson, Centereach, Smithtown, Coram, and surrounding Suffolk County communities receive comprehensive foot and ankle care, including advanced diagnostics, custom orthotics, digital X-rays, sports medicine, wound care, surgical care, EPAT shockwave therapy, and treatment for a full range of podiatric conditions.

What Is Diagnostic Ultrasound?

Diagnostic ultrasound is a non-invasive imaging test that uses high-frequency sound waves to create real-time pictures or video of soft tissues inside the body. Unlike X-rays, ultrasound does not use ionizing radiation.

In podiatry, diagnostic ultrasound may be used to evaluate painful or injured structures in the foot and ankle, especially when symptoms involve tendons, ligaments, fascia, nerves, cysts, or inflammation.

Diagnostic ultrasound may help evaluate:

  • Plantar fasciitis
  • Achilles tendonitis
  • Peroneal tendon injuries
  • Posterior tibial tendon dysfunction
  • Ligament sprains
  • Neuromas
  • Ganglion cysts
  • Bursitis
  • Soft tissue masses
  • Foreign bodies
  • Joint inflammation
  • Tendon tears
  • Scar tissue
  • Sports injuries
  • Chronic foot or ankle pain

Diagnostic Ultrasound vs. Digital X-Rays

Both imaging types are valuable but used for different purposes. Digital X-rays are best for bones and joints — fractures, arthritis, heel spurs, bunions, hammertoes, bone alignment. Diagnostic ultrasound is best for soft tissues — tendons, ligaments, fascia, neuromas, cysts, bursitis, and inflammation.

Comprehensive Podiatry Associates offers in-office digital X-rays for fast evaluation of bone and joint conditions. Diagnostic ultrasound can complement X-rays when the suspected problem involves soft tissue rather than bone.

Conditions That May Be Diagnosed With Foot & Ankle Ultrasound

Plantar Fasciitis — Ultrasound may help evaluate thickening, inflammation, or degeneration of the plantar fascia, plantar fascia partial tearing, chronic inflammation, heel fat pad changes, fluid near the fascia, and other causes of heel pain.

Achilles Tendonitis — Diagnostic ultrasound may help evaluate Achilles tendon thickening, inflammation, degeneration, partial tearing, or fluid around the tendon.

Tendonitis & Tendon Tears — Ultrasound can evaluate the Achilles tendon, posterior tibial tendon, peroneal tendons, extensor tendons, and flexor tendons for tendonitis, partial tears, tendon sheath inflammation, and chronic overuse injuries.

Neuromas — Diagnostic ultrasound may help identify soft tissue enlargement around the nerve and distinguish neuroma pain from capsulitis, bursitis, stress injury, or joint inflammation.

Ganglion Cysts & Soft Tissue Masses — Ultrasound may help determine whether a mass is fluid-filled or solid and whether additional imaging or treatment is needed.

Ankle Sprains & Ligament Injuries — If pain persists after a sprain, diagnostic ultrasound may help evaluate ligament injury, swelling, tendon involvement, or scar tissue.

Bursitis — Ultrasound may help identify fluid accumulation and inflammation in the affected area near the heel, Achilles tendon, ball of the foot, or areas of pressure from shoes.

Foreign Bodies — Ultrasound may help locate certain soft tissue foreign bodies and guide treatment planning.

Benefits of Diagnostic Ultrasound

  • Non-invasive imaging with no incisions, injections, or radiation exposure
  • No ionizing radiation — uses sound waves instead of X-rays
  • Real-time results — can show soft tissue structures during movement
  • Excellent for soft tissue problems — tendons, ligaments, fascia, nerves, cysts
  • May help guide treatment decisions toward orthotics, bracing, physical therapy, injections, EPAT, immobilization, wound care, or surgery

Who May Need Diagnostic Ultrasound?

You may benefit from a diagnostic foot or ankle ultrasound if you have heel pain that has not improved, suspected plantar fasciitis, Achilles tendon pain, tendonitis or possible tendon tear, ball-of-foot pain, burning or tingling between the toes, a lump or soft tissue mass, chronic ankle pain after a sprain, swelling around a tendon or joint, pain that does not show clearly on X-ray, sports-related foot or ankle pain, or recurring soft tissue inflammation.

What to Expect During a Diagnostic Ultrasound Appointment

Step 1: Your podiatrist reviews your symptoms, medical history, activity level, footwear, injury history, and prior treatments.

Step 2: Your podiatrist determines whether ultrasound, X-rays, or another imaging option is most appropriate.

Step 3: A small handheld transducer is moved over the painful area with ultrasound gel, sending sound waves into the body and creating images of the tissues below the skin.

Step 4: Based on findings, your podiatrist may recommend custom orthotics, supportive footwear, bracing or splinting, stretching, physical therapy, EPAT shockwave therapy, cortisone injection, lubricant gel injection for arthritis, wound care, or surgical evaluation if needed.
